If the error (code: 0004-0004) appears after the pre-upgrade check, go to Policies > Access Control > Local ACL and check if any custom local ACL policy is named default-allow or default-deny (case-sensitive). Delete these policies or rename them, then reinstall the pre-upgrade check package.
